Given the side effects of Cipro include diarrhea, dizziness, headache, stomach upset, abdominal pain, nausea/vomiting, and rash, how does one know if it is actually working while eating, living in India?

I have a good friend who used to go there a lot for work. He said you could be in the nicest hotel you've ever seen then walk two blocks and be in the filthiest, smelliest sh*thole imaginable.
