See a medication specialist. There are a lot of people on SSRI type medication who actually are bi-polar (not the stark raving mad type of bi-polar either, bi-polar 2 which is often unknown to the person) and your Zoloft type medications either don't work or make the problem worse. If you live anywhere near a big city it shouldn't be a problem finding someone who can dial something in for you. I have the same issues, been that way since I was small. When I get depressed, I start taking B12 vitamins, they help a bunch. Go to a Health Food store and find some, I don't think they are too expensive. I found those helped way more than anything the doc gave me.
Also, as others have suggested, go get some professional help. It's nice to have someone to talk to that won't judge you. That also helped me when all my family would say is "snap out of it." Like it's some kind of switch I can turn pff and on? Okay.

I have never been on a prescribed medication, but things that I have found that help are: most importantly regular exercise, consistent sleep pattern, and eating healthy (including limiting alcohol & caffeine). There are also natural supplements which can help you relax... 5-Hydroxytryptophan (marketed as 5-HTP) and L-theanine. Also, getting regular sunlight can help...and if you suffer from SAD as well there are therapy lights for that.
If you ever have trouble motivating yourself to work out or to do any of the proactive things to help you, just remind yourself how you feel after you do the things that help and compare it to how you feel at that moment when you lack motivation. I'm not big on medication, though I understand why it is used. I think (in most cases -- I understand depression may be caused by organic factors) drugs treat the symptom but don't cure the disease. Also, because no drug works for everyone and because there is no way of telling what drug will work, taking the drugs until you do find out which one works for you can be a dismal experience.
I think that with depression, like any disease, you need to find the cause and deal with it. I recommend therapy. You may or may not be able to isolate what in your life is causing you to be depressed, but therapy will help you deal with it.
I agree with those who suggest you work on things in your life to increase your self-esteem. That alone may be able to break the cycle for you. Good luck.
